The ideal candidate for the Recreation Aide role is a caring and compassionate individual that is dedicated to making a positive impact on their residents. Learn and expand professionally in a supportive, collaborative environment.

The Recreation Aide’s specific responsibilities include:

  • Assists in planning and promoting recreation based on the needs and interests of the customer.
  • Provides programs in all recreation component areas on an individual and group basis.
  • Sanitize all supplies after each activity program. 
  • Assist customers with leisure materials and programs.
  • Documents resident attendance/snacks/fluids offered in plan of care and participations level at recreation programs. Attends care plan meetings as appropriate.
  • Performs other responsibilities as directed by the Recreation Director.

The Recreation Aides must be:

  • Caring, compassionate, and strong outgoing personality.
  • Creative in Art, Music, Crafts preferred 
  • Experience in similar roles preferred but willing to train
  • Ability to read, write, speak and follow written and oral directions in the English language.
  • Bilingual in Spanish or any other language a major plus!
  • Be able to comply with any and all federal, state, city, and Rebekah rehab security and privacy policies intended to protect the privacy of individual identifiable health information.
  • A Self starter

Recreation Aide

Alternating weekends, weekdays, evenings, holidays

Sunday coverage for other days off.

The Recreation Aide’s specific responsibilities include:

  • Assist residents with leisure materials and programs.
  • Documents resident attendance/snacks/fluids offered in plan of care and participations level at recreation programs. Coverage of charting when full time staff are on vacation, holidays. Attends care plan meetings as appropriate, will train the right candidate.
  • Performs other responsibilities as directed by the Recreation Director.

SOCIAL SERVICES DEPARTMENT

POSITION TITLE: Social Service Secretary/Residents Service Coordinator

DEPARTMENT: Social Service/Administration

SUMMARY OF MAJOR FUNCTIONS:

Secretarial and support functions needed to service social work operations. Support functions inherent in coordination of effective residential services. Liaison between residents and various departments in order to facilitate service on behalf of the resident.

POSITION ACCOUNTABILITY:

The Social Service/Resident Services Coordinator is responsible to the Director of Social Services.

POSITION QUALIFICATIONS:

  1. Bilingual (Spanish speaking)
  2. High School Graduate
  3. Ability to type, file, use word processor, computer skills and other secretarial duties
  4. Effective verbal and written communication skills
  5. Organizational skills
  6. Excellent interpersonal skills necessary to effectively work with residents
  7. Ability to function independently and have flexibility

DUTIES & RESPONSIBILITIES:

  1. Separates, reads and distributes mail within the Social Service Department.
  2. Answers and screens telephone calls, takes appropriate messages
  3. Prepares and distributes all inter-office correspondence re: Resident leave of Absence, discharge notice, room changes (including door & bed labels), etc.
  4. Prepares confidential reports as necessary, i.e.: resident council minutes, etc.
  5. Prepare letters regarding residents i.e.: current location letter, LT vs ST placement, family letters
  6. Perform secretarial duties, i.e.: order office supplies, soap, paper towels (keep appropriate supply on hand), make copies(keep appropriate supply on hand), scan and upload into medical record/drive
  7. Acquires knowledge of the department and uses judgement regarding social service code of ethics, policies and procedure to ensure orderly operations of the department
  8. Complying with HIPPA regulations regarding confidential resident information as we as staff in the release of information
  9. Responsible for social service files and their upkeep
  10. Keep appropriate records necessary to meet Social Service state code of regulations, i.e., room transfers, grievance book, advance directive binders
  11. Schedule meetings, make appointments, answers questions and give information based upon knowledge of department organization and regulations.
  12. Sends out letters/emails to families informing them of interdisciplinary family conference and keeps record of responses to be provided Director prior to meeting.
  13. Keeps abreast of changes and trends related to responsible functions.
  14. Provides residents/designated representative of appropriate information re: personal funds, amount of leave currently available, clothing p&p/laundry, etc.
  15. Send out/email quarterly bank statements to designated representatives of cognitively impaired residents. Provide cognitively intact residents with their banks statements.
  16. Gathers information necessary and inform Social Service Director/ Assigned Social Worker.
  17. Enter information into MDS for new residents/re-admission as needed.
  18. Update new face sheet into sigma as changes occur
  19. Update smoking list as needed (when smoker is added/removed) and distribute updated list
  20. Keep Social Service Bed Board updated
  21. Responsible for ordering/shopping clothing and shoes for residents (communicate this with assigned social worker for appropriate documentation).
  22. Emails discharge notice notices to assigned social worker
  23. Prepare and oversee Clothing Store twice per year. Submit appropriate information needed to Finance office.
  24. Complete Admissions Agreement; send email to appropriate party (MF) and Director if there are any concerns.
  25. Maintain database information on excel spreadsheet of discharges and complete 30day follow up call. Report to Director of re-hospitalization occurred.
  26. Make rounds to ensure door and bed labels are present.
  27. Complete rounds as needed.
  28. Other related duties as needed.
