We take great pride in providing compassionate and comprehensive medical care – rendered as much as possible on-site at our facility to avoid unnecessary hospitalizations and ensure quality care for our patients.

The Team Approach:

Our short and long-term therapy and rehab programs are delivered by a full-time staff of highly trained, multi-lingual team members who participate in a team approach to patient care.

  • Initial consultation is conducted.
  • Individualized treatment program is designed.
  • Patient receives individualized care from a rehabilitation therapist and an assistant therapist. This team remains with the patient throughout their recovery to ensure consistent and effective care. 
  • Rehabilitation care team collaborates with other interdisciplinary teams, including nursing, medicine, social services, nutrition, and recreation, to ensure that all the patient’s needs are being met through our comprehensive range of services.
  • Focused and comprehensive rehab services are conducted up to 3 hours/day, 7 days/week to deliver maximum benefits and results based on patient condition and necessity. 

Our medical care team of experienced specialty physicians provides compassionate care for our patients. An integrated care model provides on-site care, allowing our patients to avoid time consuming transfers, lengthy waits in doctors’ offices, and potential delays in implementation of recommendations. Our unique model for rehabilitation, sub-acute, post-hospital, and long-term geriatric care ensures that our patients have direct access to physicians specializing in their unique needs.

Our Primary Physicians:

  • Are selected based on their demonstrated interest and expertise in caring for patients with sub-acute, post-hospital, long-term care, and rehabilitation needs.
  • Have over 15 years of clinical experience providing care in similar settings. 
  • Are board certified or board eligible and many have completed fellowship programs in geriatrics or other relevant medical specialties. 
  • Work closely with our specialists in the community or the hospital setting to ensure continuity of care and prompt return to our facility for continued care. 
  • Visit Rebekah on a regular schedule during the week to maximize interaction with staff, family members, and most importantly, our patients.
  • Are available to meet with families to review medical care and progress. 
  • Works under the supervision of a medical director with 20+ years of experience in this care setting and are on-call 24-hours/day.
  • Participate in a comprehensive multidisciplinary approach to falls preventions and evaluation for every individual who falls with a specially trained team, including nursing staff, physical and occupational therapists, and other health care team members with specific expertise.
  • Take part in other important patient care activities including pain management, rehab and post hospital care planning, advance directive planning and implementation, discharge planning, and when appropriate, end of life care.
  • Provide on-site radiology services, EKG and Holter monitoring, vascular and ultrasound testing, blood testing, and dialysis.

Our highly trained nursing department professionals provide personalized care for each of our patients 24-hours/day, 7-days/week to enhance their optimum health, comfort, independence, and well-being.

Nursing Specialists, including Nurse Practitioners, RNs, LPNs, and Certified Nursing Aides & Orderlies:

  • Work closely with interdisciplinary care teams, including physicians, rehabilitation specialists, nutritionists, social workers, and recreation therapists to establish and provide each patient with individualized care in a supportive and caring environment.
  • Offer expertise in important disciplines including care planning, infection control, staff education and quality improvement initiatives.
  • Provide ongoing or post-hospital specialized care, including complex wound care, feeding tubes, tracheostomies, and the use of intravenous and PICC lines.

Our Certified Nursing Attendants (CNAs) provide direct care to our patients. As a vital part of our care team, CNAs:

  • Attend to the needs of multiple patients
  • Aid patients with routine tasks, such as eating, bathing, dressing, and exercise
  • Observe and record patient conditions and vital signs
  • Monitor patient food and liquid intake
  • Update the care team on changes in patients’ conditions

Our Physical & Occupational therapists work with inpatient, outpatient, and home rehab patients. So, if you spend time in our rehab facility following a hospital stay, you’ll enjoy care from familiar faces when the same therapists come to your home to help you recover.

The Rehabilitation Center

Inpatient and outpatient therapy and rehabilitation services are delivered in a 3,500 square-foot newly constructed therapy gym filled with the most modern and comprehensive therapy equipment available in the area. The Rehabilitation Center includes treadmills, stationary bikes, practice stairs, and other modern therapy equipment, along with a simulated independent living (ADL) suite. The ADL Suite, complete with a full working kitchen, dining room, living room, bedroom, and bathroom, is used by patients returning to the community to practice independent living skills. The Suite is proven to dramatically increase the success rate of independent living, preventing rehospitalization.

Physical Therapy

Helps patients regain physical mobility, strength, balance, and endurance. 

Expert Physical Therapists: 

  • Use treatment methods designed to assist patients with sitting and standing balance, ambulation, stair climbing, and wheelchair mobility. 
  • Provide patient and family education to ensure a smooth transition from rehab facility to home or next living environment. 
  • Assist patients in acquiring necessary equipment for discharge and providing instruction on the equipment’s proper usage.

Develop a written home exercise program to be used after discharge, as needed.

Occupational Therapy

Enhances the skills needed for individuals to achieve independence in their daily life. 

Certified Occupational Therapists:

  • Promote independence in all areas of living, including health and hygiene. 
  • Provide patient and family education to ensure a smooth transition from rehab facility to home or next living environment. 

Speech and Language Therapy
Meet the diagnostic and therapeutic needs of any inpatient experiencing communication and swallowing/feeding disorders. 

Master’s Level Speech/Language Pathologists:

  • Structure care to meet the individual needs of each patient.
  • Work closely with patients, families, and rehab team members to achieve maximum results.  

Specialized Programs

In addition to the basic treatment modalities, we also offer a full complement of specialized rehabilitation services, including:

  • Arthritis Rehabilitation – Combining personalized care and cutting-edge technology to help restore maximum mobility and functionality.
  • Back & Neck Rehabilitation – Alleviating the pain and disability associated with degenerative back and neck disease and other conditions.
  • Orthopedic Rehabilitation – Individualized treatment for a variety of hip, knee, bone and joint disorders.
  • Continence Training – Helps patients with impaired bladder and bowel function to achieve increased continence levels through diet, medications, surgical intervention, training and fluid management.
  • Diabetic Foot & Wound Care – An inter-disciplinary program dedicated to the treatment and prevention of diabetic foot disease and wound care problems.
  • Lymphedema Management – Designed to help alleviate the pain and discomfort caused by fluid retention in the extremities as a result of compromised or excised lymph nodes, the program focuses on therapeutic manual lymph drainage (MLD) techniques.
  • Stroke Rehabilitation – Comprehensive inpatient and outpatient services for stroke patients and their caregivers.
  • Cognitive/Linguistic Treatment – A therapeutic system designed to help patients with brain injuries, or other disorders that compromise cognitive function, better manage daily activities.
  • Dysphagia Program – An integrated approach to rehab for patients experiencing swallowing disorders, designed and implemented in conjunction with speech and language therapy professionals.
  • Post Therapy Wellness – Designed to help patients who complete an ambulatory rehab program maintain function and continue their progress following discharge.
